This service supports clients aged 17years or under receiving clinical services from the ADHB Kari Centre who reside in the Auckland District Health Board catchment area and would benefit from support in the home and/or community.

Counties Manukau

Youth Community Living Services

This service supports clients aged 13 -18 years with high support/recovery needs who reside in the Counties Manukau catchment area and would benefit from support in the home and/or community.

 

Entry by referral from Whirinaki Child Family and Youth Clinical Health Partner - Ph. (09) 265 4199

 
Advocacy/Peer Support - Whanau/Child and Youth

This service provides community and clinical advocacy and peer supports to the Whanau of clients aged 13 -18years so that they are better positioned to support their loved ones on their journey to wellness.

 

Entry may be by self-referral from a Youth Service client or his/her whanau directly, referral from primary and secondary health services, Maori health providers and community members.

Adult Services

Auckland

Kaupapa Maori Adult Support Hours

This service supports clients aged 18 years who are experiencing a mental health and addiction disorder and receive clinical services from the ADHB Community Mental Health Services and would benefit from support in the home and/or community.

This service provides support to Maori client who reside in the Auckland District Health Board catchment area.

 Entry into this service is by referral from the ADHB Clinical Services through the ADHB Local Coordination Service.

Counties Manukau

These services provide support to clients aged between 18 and 65 years who reside in the Counties Manukau District Health Board catchment area and would benefit from support in the home and/or community.

 

Community Living Services

Supporting clients who have previously lived in in-patient services or residential rehabilitation to find personalised solutions that will enable them to live full lives in their community of choice and to find and create homes for themselves.

Entry into this service is by referral from Te Puna Waiora (Community Maori Mental Health Partner ) Services - Ph. (09) 261-3700

Community Support Services

This service provides a flexible mobile community based support service for clients who are living independently and require support in relation to family/whanau, community living, education, employment and self-management of their wellbeing.

Entry into this service is by referral from Te Puna Waiora (Community Maori Mental Health Partner ) Services - Ph. (09) 261-3700 or by self-referral for previous users of the service.

Iwi Support Services

Iwi Support provides a recovery orientated community based service within a Kaupapa Maori framework to support clients in their individual journey to wellness.  This service is specifically targeted towards Maori clients and/or clients preferring a Maori centred pathway of care.

Entry by self-referral, referral from primary and secondary health services, family, carers and/or community members

Mobile Community Support Services

This service works to build people’s own strengths and networks to enable them to live full and participatory lives within the community, achieve their goals in terms of work, friendships and family connections and to enhance their ability to complete activities of daily living that increases wellbeing rather than fostering reliance on formal mental health services and supports.

Entry to this service is determined following assessment by the ICLS Provider and relevant DHB clinical team who deem someone as likely to benefit from mobile community support.

Housing and Recovery Services

The service aims to help people move towards more independent living or to retain their level of independence in the community. Tenants are supported to actively participate in the full management of their homes and in the community.

Other supports include services to increase access to:

  • regular clinical and community support
  • cultural support
  • support to clinical appointments
  • daily living and leisure activities
  • training opportunities toward employment, community participation and social integration
  • meaningful employment opportunities
  • community resources that may improve and promote independence.

Cultural Support

 

We believe for us to be an effective provider of Kaupapa Maori Services, the involvement of Kaumatua is essential

It is through wisdom, experience and guidance from Kaumatua on those aspects which relate to Te Ao Maori and Maori culture that enable us to ensure service delivery to Maori, tangata whaiora and whanau continues to remain culturally safe and of a high quality standard.

Supports include:
  • Ongoing advice and information in regard to aspects of care such as assessments and treatment options or approaches/activities that reconnect people with whanau, hapu and iwi

  •  Assistance in facilitating further interventions or refer to appropriate roopu/agency/services for tangata whaiora

  • Provision of or access to a Maori environment, rongoa Maori, tohunga, Te Reo Maori speakers or other cultural needs in accordance with the needs/requests of tangata whaiora and whanau

  •  Provision of Tikanga Maori that relate to the kawa and kaupapa of the service. This may include: Powhiri, Mihimihi, karakia, Waiata, hui or Poroporoaki
